Child Development Program at HCS

Application for child development

Your child must be 4 years old, on or before, September 1st to participate in the CD program.

You will need the following documents to apply: child's legal birth certificate, shot record, Medicaid card for child, or parent proof of income (tax return or W-2), proof of residency (utility bill, rental or lease agreement).  Your child's social security card is requested, but not required.
